Home Health Agency Partnership Development Guide

MHA’s Home Health Agency Partnership Development Guide assists hospitals’ efforts to reduce readmissions and improve patient experiences through strong, informed relationships with post-acute care providers.

The guide is modeled on the Skilled Nursing Facility Partnership Development Guide (released in May) and provides invaluable qualitative information, in a single publication, that supports hospitals’ efforts to identify, develop, and strengthen formal and informal partnerships with home health agencies. Similar to the SNF guide, this publication contains data on all Medicare-certified agencies and can be easily navigated by county and HHA name. If you want to print this document, please keep in mind the guide is over 100 pages long. The document is intended to provide objective information from a variety of sources in a single place and includes:

  • a discussion guide to facilitate communication in the interest of developing high-quality and cost-efficient care
  • INTERACT Tool that HHAs could be asked to complete to highlight their capabilities
  • key quality and cost measures, including all-cause readmissions rates

Both the Home Health guide and the SNF guide are most effective when used with another new tool MHA has developed. The Post-Acute Care Market Profile enables hospitals to gain a clear picture of not only which partners they are referring patients to, but also how much referrals to individual SNFs and HHAs contribute to their market shares. This is an Excel document, and each worksheet contains Medicare data for a different hospital; the front tab is an index sheet for easy navigation. This information is useful in better understanding your organization’s relationship to its post-acute partners and may be used to: request regular meetings, encourage greater collaboration/information sharing, and develop shared innovative procedures.
