Job Summary
The Association is seeking a Director, Population Health to join our team in Elkridge, MD. Reporting to the Senior Vice President, Care Transformation & Finance, the Director will collaborate closely with colleagues and hospital leaders to execute MHA’s advocacy goals for Maryland hospitals on issues related to care delivery transformation and population health. This position is responsible for working with MHA members (executive, physician, nursing and operations leaders) to lead population health improvement strategies under Maryland’s partnership with the Centers for Medicare and Medicaid Services arrangements including the new States Advancing All-Payer Health Equity Approaches and Development (AHEAD) Model that will launch in Maryland.
Essential Duties and Responsibilities
- In close coordination with the Senior Vice President, support members in creating, shaping, and advancing evidence-based health improvement strategies across the full healthcare continuum including primary care, chronic disease management, emergency care, and post-acute services.
- Facilitate MHA member workgroups and learning collaboratives to identify conditions driving avoidable utilization, readmissions, and/or cost within the hospitals/health systems and support members in identifying solutions by providing data analysis, best practices, and research.
- Represent MHA and its member hospitals/health systems in state agency led policy workgroups for population health planning and health improvement focus areas (e.g., maternal health, diabetes, behavioral health, etc.) and disseminate relevant information to member hospitals/health systems.
- Establish and foster constructive working relationships with diverse stakeholders including MHA member hospitals and state agency staff in areas related to population health programming
- Collaborate with MHA’s Strategic Analytics team to create data driven analyses that support member hospital advocacy positions and policy recommendations
- Monitor and maintain understanding of significant state government activities in population health as the AHEAD Model is implemented in the state
- Create documents that represent the Association to members and the public, such as meeting presentations, background memos, comment letters, talking points, etc.
- Create and update work plans; execute on next steps identified during meetings; follow-up with individuals to understand and resolve concerns and barriers; accept and manage ambiguity and complexity in reaching objectives
Education and Experience
- Bachelor’s degree with a minimum of 5 years relevant experience supporting health programs required
- Master’s degree preferred
Qualifications
- Working knowledge of, or ability to quickly learn population health improvement strategies required under the goals of Maryland’s AHEAD Model developed by the Centers for Medicare and Medicaid Services
- Ability to build strategic relationships across a wide range of stakeholders including with internal MHA teams that support the work and with external hospital executives, administrative staff, clinicians, and government agencies
- Strong presentation skills with the ability to facilitate hospital workgroups including the development of presentation content, key discussion questions, and capturing of necessary follow up steps related to key population health initiatives
- Ability to understand health care issues and challenges, relate them to MHA and stakeholder needs, and propose appropriate solutions
- Strong Excel, Word, and Power Point skills to organize and display information to facilitate discussion and decision-making
- Strong writing skills and ability to synthesize qualitative and quantitative data, summarize policy in simplified, ‘lay-person’ language supported by easy to interpret information
- Active listener with the ability to adapt communication style to audience
